What is an Impacted Canine? How is it Treated?

An impacted canine is when the tooth fails to emerge into the oral cavity on time and remains embedded within the bone. After wisdom teeth, canines are the most commonly impacted teeth, occurring twice as frequently in females compared to males, affecting about 2% of the population. Impacted canines are twice as common in the upper jaw compared to the lower jaw and are unilateral in 90% of cases, with 10% being bilateral.

**Dangers of Impacted Teeth**

Impacted teeth can erode the roots of neighboring teeth: Scientific studies have shown that misaligned erupting canines can erode the roots of permanent incisor teeth between the ages of 10 and 13.

They can lead to dentigerous cysts: These are benign lesions that may form due to development or inflammation.

They can cause pain: Pain may develop related to the impacted tooth.

**Symptoms of an Impacted Tooth**

- The tooth has not emerged despite its symmetrical counterpart having done so.

- If the root of the deciduous canine has not started resorbing by age 12.

- Absence of canine bulge in the jawbone at the lip side (vestibule) by age 12.

- Presence of a deciduous canine that has not shed by age 13.

- Swelling or bulging in the palate (superficial impaction symptom).

- Missing or malformed upper lateral incisors (sometimes).

- Gaps or misalignment of the front lateral incisors.

- Non-resorption of the deciduous canine root in a timely manner.

- Failure of the deciduous canine to shed due to fusion with the bone.

- Individuals with cleft lip and palate are at risk.

**Causes of Impacted Canines**

If the impacted canine is palatal (on the palate side), genetic factors and congenital tooth absence are more influential; if it's buccal (cheek side), early loss of deciduous teeth, mismatch in dental arch width, and crowded teeth play a larger role.

Premature extraction of deciduous canines creates space issues.

If there's less than a year until the natural shedding time of the lower teeth, the eruption of the permanent tooth accelerates; however, if more than a year remains, the overlying bone hardens, delaying eruption and potentially leading to impaction.

**How is an Impacted Canine Diagnosed?**

An orthodontist can easily diagnose impacted canines early on through periapical X-rays, panoramic X-rays, or cone-beam computed tomography. Early diagnosis, as in all fields of healthcare, is crucial in orthodontics too. The American Association of Orthodontists recommends that children have their first orthodontic examination at age 7.

**Can an Impacted Canine Erupt on Its Own?**

No, impacted canines do not erupt on their own and remain impacted.

**What Happens if Impacted Canine Treatment is Not Performed?**

The canine tooth, having the longest and strongest root in the body, will attempt to erupt into the oral cavity over time but will fail. During this process, its durable enamel structure will repeatedly hit the weaker roots of neighboring milk and permanent teeth, inevitably resorbing and eroding them. If orthodontic treatment is not performed, an impacted canine can hinder treatments like implants or dentures intended for tooth replacement later on. Correction of impacted canines is safely possible in many cases.

**How is an Impacted Canine Treated?**

The preferred treatment approach for impacted teeth is early diagnosis and intervention. However, early diagnosis without an orthodontic examination can be challenging. A common misconception is that orthodontic treatments can only be performed after ages 12-13.

In early stages (ages 7-11):

We prevent space loss through orthodontic space regaining procedures. In suitable cases, this can be done by moving all teeth backwards (distalization) or by widening the jaw, which can also be performed with Invisalign clear aligners or

 palatal expanders.

For your impacted teeth to erupt naturally, we implement treatment to remove the obstacles in front of them and monitor the process. In many cases, we can resolve the issue naturally during the early stages.

In later stages (ages 12+):

1) Position identification: X-ray diagnosis of the impacted tooth.

2) Surgery for palatal impacted canine: In our clinic, we remove the surrounding soft tissue with a soft tissue bur or laser painlessly. We then perform either closed or open eruption techniques depending on the clinical situation and our treatment plan.

3) Tissue healing: We wait three days for healing. If a protective cap was placed, we remove it.

4) Eruption of the impacted tooth: We attempt to bring your tooth into the oral cavity naturally or with orthodontic force.

Is Surgery for an Impacted Canine Difficult?

No, the procedure is performed under local anesthesia, ensuring complete numbness and no discomfort.

Duration of Surgery for an Impacted Canine

The operation can last from 5 to 45 minutes, depending on the position and angle of the tooth.

What Happens if an Impacted Canine is Not Treated?

If not treated in time, impacted teeth can lead to the aforementioned undesirable outcomes. The probability of successfully erupting a tooth at ages 12-13 compared to ages 20-25 is not the same; the likelihood decreases with age. However, regardless of how advanced your age or how severe the problem, you should consult an orthodontist and be willing to have your tooth erupted. At Hero Orthodontics and Aesthetic Dental Clinic, we are always ready to assist you. Otherwise, the tooth will inevitably need to be extracted, and no prosthesis can ever truly replace the original.

Duration of Eruption for an Impacted Canine

The duration can vary depending on the position and location of your impacted tooth. While some impacted teeth can be erupted within 2-3 months, others may take 12-18 months.

A responsive tooth will make itself known within 3-4 months. If after 6 months, no movement is detected in the radiograph, it may be necessary to discontinue efforts and consider surgical extraction.

Can Braces Be Applied with an Impacted Tooth?

Yes, braces can be applied even with an impacted tooth. In many of our orthodontic treatments, we combine treatment for the impacted canine with braces to save time and shorten the overall duration of treatment. While creating the ideal space for your impacted tooth to erupt, we continue the orthodontic treatment of your other teeth.

Positioning of the Impacted Tooth in the Dental Arch

While it is possible to erupt your impacted tooth without braces or clear aligner therapy, it isn't very practical. We can predict where your impacted tooth will erupt and roughly guide it, but functional and aesthetic benefits are only achieved when the tooth is precisely positioned in its anatomically correct place. Therefore, the primary goal in the treatment of an impacted tooth is to bring it into the oral cavity. Positioning the tooth in the ideal anatomical location in the dental arch is only possible with braces or clear aligner therapy.

Cost of Braces for an Impacted Tooth

Your impacted teeth are your own original teeth that should be in your mouth. If extracted without trying orthodontic treatment, you lose the opportunity to benefit from them.

Canines are corner teeth in our mouths, and extracting them can make an individual appear 5-10 years older since the corners of the mouth will sag without support. Therefore, the opportunity to use your original tooth after the procedure should not be unfairly compared to lifeless and sensation-less implant treatments and their costs. The cost of braces for an impacted tooth varies depending on the position and angle of your tooth. We invite you to our Hero Orthodontics and Aesthetic Dental Clinic to assist you with radiographic, 3D digital intraoral scanning, and provide a cost estimate.

Extraction of an Impacted Canine

Never have your tooth extracted without an orthodontic consultation and without your orthodontist's approval for extraction of your impacted canine tooth. If no orthodontic treatment attempt has been made, I recommend trying it first. However, in rare cases, extraction may be considered if your tooth is positioned very deeply, follows a horizontal trajectory, or is upside down. In such cases, if you have a current X-ray taken within the last three months, you can send it to info@kahramanortodonti.com.tr and always seek our help. We are pleased to assist you at our Hero Orthodontics and Aesthetic Dental Clinic in Istanbul, Bakırköy.

What Happens After an Impacted Tooth Extraction?

Follow your doctor's recommendations for the healing of the wound area after an impacted tooth extraction. Avoid hot food for a while. Do not smoke to prevent alveolitis. Seeing blood on your pillow for the first 1-2 nights you sleep is completely natural. You may see blood in your saliva, but do not spit repeatedly; swallow instead, as suction will not stop the bleeding. Applying cold is still one of the most beneficial treatments. Applying a cold pack for 15 minutes with 20-30 minute breaks during the first 12-18 hours can help control and minimize swelling. You should be carefully monitored during this period. If stitches were placed, they should be removed by a doctor within 7-10 days. Some cases may use self-dissolving stitches that disappear within 3-5 weeks. You can brush and clean your teeth after the first 24 hours. If cleaning is not done properly, an infection can develop.

What Happens If an Impacted Canine Is Not Extracted?

First, an attempt should be made to orthodontically erupt the impacted canine tooth. If your orthodontist decides on extraction, it should be extracted. If extraction is indicated but not performed, you will inevitably face:

· Your neighboring tooth roots may erode.

· Unwanted cystic formations may occur.

· You may unexpectedly face insidious pains.

Biocompatible: Emax porcelain is a biocompatible material. These veneers are compatible with the gums, minimizing the risk of allergic reactions.

Emax veneers are a widely preferred treatment method in aesthetic dentistry. These veneers improve the natural and aesthetic appearance of the teeth while providing long-lasting and durable results.

What is a Flapless Implant?

A flapless implant is a dental implant procedure performed using a minimally invasive technique, which differs from traditional dental implant methods. This method does not require the cutting and lifting of the gum tissue (flap), which accelerates the healing process and offers patients a more comfortable treatment experience. DentMax Dental Clinic, in Istanbul, Turkey, and Balıkesir, Turkey, provides flapless implant treatments, offering modern and effective solutions to its patients.

How is a Flapless Implant Performed?

Flapless implant treatment is performed using minimally invasive surgical techniques and advanced technologies. Here are the steps of this treatment process:

Planning and Preparation:

During the initial examination, your dentist evaluates your oral and jaw structure.

Detailed planning is done using digital imaging techniques (CBCT) and digital scans to determine the exact placement of the implant.

This planning ensures the implant is placed in the correct position and angle without the need for cutting the gum tissue.

Minimally Invasive Surgery:

During the flapless implant procedure, a small hole is made in the area where the implant will be placed, without lifting the gum tissue.

This small hole allows the implant to be placed directly into the jawbone with minimal disruption to the surrounding tissues.

Implant Placement:

The placement of the implant is a procedure that requires high precision.

The implant is carefully placed according to the planned position and angle.

Correct positioning of the implant accelerates the healing process and ensures the implant's longevity.

Temporary Prosthesis:

After placing the implant, a temporary prosthetic tooth can be attached to use during the healing process.

This allows patients to be comfortable both aesthetically and functionally.

Advantages of a Flapless Implant

Flapless implant treatment has many advantages:

Quick Healing: Being a minimally invasive method, the healing process is faster, and the absence of gum tissue cutting makes wound healing more comfortable.

Reduced Pain and Discomfort: Due to the small hole made without lifting the gum tissue, there is less pain and discomfort compared to traditional methods.

Less Swelling and Bruising: With less tissue damage, the risk of swelling and bruising is lower.

Quick Procedure: The flapless implant procedure can be completed in a shorter time compared to traditional methods.

Aesthetic Results: Temporary prosthetics allow patients to maintain an aesthetic appearance throughout the healing process.

Conclusion

DentMax Dental Clinic, located in Istanbul, Turkey, and Balıkesir, Turkey, offers flapless implant treatments to provide patients with comfortable and fast solutions. Flapless implants are applied using minimally invasive techniques and modern technologies, which speed up the healing process and cause less pain and discomfort for patients.
